Mannford EMS launching at-home care program to cut hospital visits

Mannford EMS is developing a community paramedicine program to provide in-home medical care, aiming to reduce hospital visits, cut costs, and improve support for patients with chronic conditions.

Mannford EMS leaders are working to launch a new program that could bring medical care directly to residents’ homes, aiming to reduce avoidable hospital visits and save patients money.

The initiative, called community paramedicine, relies on specially trained paramedics who provide at-home services like medication checks and chronic disease management—all at a lower cost than emergency room care.

Bringing Care Home

Mannford EMS Director Shane Cox and a fellow team member recently earned certification in community paramedicine, a growing model that integrates primary care, mental health, and social work into home visits.

Cox says the goal is to prevent repeat hospital admissions, particularly for patients with chronic conditions like COPD, by ensuring they’re following treatment plans and receiving necessary support.

Why It Matters

The EMS team reports that call volume has doubled over the past three years, with many calls tied to repeat issues that could be addressed through proactive home care.

“Community paramedicine’s not real big in the U.S. right now,” Cox said. “Internationally, it’s gained a lot of progress, but it’s not something we see here. So it’s big for us as a small town to bring it to Mannford — it’s not something you typically see.”

The Funding Challenge

Launching the program will require about $40,000 in the first year to cover lab equipment, a cardiac monitor, medications, and additional staff.

Cox says Mannford EMS is working with local businesses and the community to raise funds and is seeking donations to help make the service available within the next 12 months.

Who Qualifies?

To access the program, patients would need a referral from a primary care provider or social worker. Once connected, they could receive at-home visits and, in some cases, medication delivery if they’re unable to visit a pharmacy.

What’s Next?

If successful, Cox hopes to expand the program beyond Mannford to serve other communities in Green Country.
